Squid and Lug is the most popular combination. For hook size I think t all depends on how you present your squid, whether you remove the tentacles, lower body. But if you're using a whole squid, I was told you could get away with using size 4/0 hooks. Still this is coming from someone that has only ever caught Mackerel, Baby Pollack and Dogfish by rod and reel crying

Squid plain on its own will take cod but to have the edge or if visiting a new location always bring some peeler crab with you.

all of my cod caught in the Bristol channel have fallen to lug worm either live or frozen.
peeler crab and squid will also do the business.
Either use the lug on its own or in a cocktail.
Ragworm will catch you fish but it isn't the best bait for cod

Peeler crab works well for cod/codling during autumn and spring, but during the main winter months lugworm, herring and squid are by far the best baits to use.

Tight Lines.
